City Care is searching for the right person with a knack for seeing great potential in others and working as part of the team with the shared goal of motivating change toward that potential. The right candidate will be organized, task-oriented and well-equipped to work with families with children and vulnerable single women and men on a flexible schedule.

The City Care Family Housing Navigator’s primary role is to:

Assist families with in the shelter who are experiencing homelessness meet their goals and objectives within a psychosocial rehabilitation framework that allows for successful linkage to housing resources in Oklahoma City and successful housing stabilization.

Responsibilities include but are not limited to:
  • Carry out the mission, vision and values of City Care.
  • Know the resources available in the community and develop a network of contacts.
  • Coordinate school enrollment for children accompanying families.
  • Develop individualized plans with families by working to identify their self-directed goals.
  • Improve guests outcomes by collaborating to develop, monitor, and evaluate service plans.
  • Provide case management assistance, resource development, crisis intervention and direct support to families in the shelter (including document recovery, collaboration with OKC’s Continuum of Care and other partner agencies to leverage resources and facilitate progress and forward motion).
  • Promote housing stability for each resident by documenting progress, referrals and contacts in the electronic record.
  • Ensure ROI’s, case notes and weekly documentation is updated and in compliance with best practices.
  • Educate new incoming families about rules and policy in the family suites. Enforce policy as needed to ensure a safe and dignified shared community space.
  • Manage donation inventory in family suites and collaborate with Volunteer Coordinator to replenish stock when needed. (diapers, clothing, laundry detergent, ect.)
  • Meet the minimum expectation of weekly face-to-face meeting with families.
  • Facilitate in-house group meetings and social activities in the community.
  • Attend weekly staffing and community meetings as designated.
  • Uphold professional confidentiality among residents, interns, externs, volunteers and staff.
Eligibility, Knowledge, Skills, Abilities:

  • Understand the complexity of the intersection of poverty, homelessness, mental illness, substance use, and recovery
  • Ability to reflect recovery principles with a minimum of one year of sobriety Skilled written and verbal communication, motivational interviewing
  • Ability to build genuine rapport quickly
  • Ability to multitask and stay cool under pressure
  • Ability to work independently while maintaining a high level of accountability Ability to work as a member of a team to accomplish shared goals
Requirements:
  • Previous experience in case management, recovery, or justice work. Or previous experience working with adults and children living with disabilities.
  • Ability to use a computer and accurately enter data into a database. HMIS experience preferred. Pre-employment background check, driving record, and drug test.
  • Must have a current Oklahoma driver’s license, reliable transportation, and updated car insurance.
Preferred:

Professional experience navigating mental and physical health systems, benefits, and resources

Previous shelter experience

Behavioral Health Case Manager II

Peer Recovery Support Specialist or previous work or lived experience in a recovery environment

Days / Hours: Full-time, Mon-Fri 11:00 am-7:00 pm (With Reasonable Flex)

Starting rate: $20.50/hour

M-F 11 am - 7 pm
