DDD Support Coordination Plan Reviewer

The Support Coordination Plan Reviewer provides documentation reviews, editing, approvals, and guidance to Support Coordinators for individuals receiving services under Division of Developmental Disabilities (DDD) through Progressive Center for Independent Living.

Support Coordination Plan Reviewer Duties:

  • Keep up to date with DDD requirements for document and writing for all annual individual plans;
  • Ensure current knowledge through DDD Policies and Procedures Manuals, DDD Updates, PCIL Plan Guidelines, DDD Best Practices Guides, DDD Update Webinars, among others;
  • Utilize DDD iRecord database to review and approve documentation;
  • Review all annual documentation for Individual Service Plans and Person-Centered Planning Tool annual renewals
  • Ensure that services are provided in accordance with the service definitions and parameters outlined by the state
  • Maintain PCIL Plan Guidelines as per DDD standards along with SC Supervisor, SC Asst. Supervisor, and SC Quality Assurance
  • Communication with Support Coordinators to edit documents and plans as per PCIL Plan Guidelines;
  • Guide Support Coordinators on questions regarding kickback review of plans
  • Maintain communication with Supervisor and Asst. Supervisor on training needs and areas of improvement for Support Coordinators
  • Maintain state and federal documentation requirements and meet deadlines;

Hours of Work:

Work Location: In person

Full time, Flexibility between Mercer and Hunterdon office locations, and work from home option possible after 90 days probationary period.

Monday through Friday, 9:00 AM – 4:30 PM; half hour lunch break (35 hours).

Flexibility is needed for Support Coordinator, state deadlines, and department needs.

Required Education and Experience:

  • Minimum of Bachelor’s degree
  • 3 years’ experience as a Support Coordinator
  • Fingerprinting, Federal and State background checks required

PCIL has been empowering the lives of people living with disabilities since 1996. We offer our four core services: information and referral, advocacy, peer support, and independent living skills. Programs available at PCIL encompass our mission to advance the independent living and rights of people with disabilities. Services available in Mercer and Hunterdon counties, with Support Coordination also available in Somerset County.

Hours: Monday to Friday, 9:00am – 4:30pm (with flexibility due to PCIL needs)

Pay: $55,000.00 per year
